We serve youth (14 – 21) and their families impacted by substance use disorder and mental health concerns.
Services include:
- Free support with or without an appointment
- Evaluating youth’s needs
- Group or individual meetings
- Talking with parents who need help/support
- Referring to other services
- Preparedness sessions for Portage Programs and Services
Our goal is to meet youth, wherever they are in their journey, and address substance use while working at resiliency, self-awareness and selfesteem.

Expanding Access to Youth Mental Health and Addiction Support
If you or your teen is struggling with substance use or mental health challenges, you're not alone—and there is help. The Portage Atlantic Community Care Program (PACCP) was created to offer local, short-term support that’s easy to access and free of judgment. We know that asking for help can be hard, and we’re here to make that first step a little easier.
PACCP offers a 14-day period of focused, short-term support designed to help youth and families manage challenges in a safe, welcoming space. During this time, we work together to explore what’s going on and build coping strategies. It’s a chance to pause, reflect, and take meaningful steps forward—with guidance and care every step of the way.
We’ve seen how much of a difference early support can make. By offering help sooner, we can prevent things from getting worse and give youth a better chance at long-term success. PACCP is here to meet you where you are, with care that’s compassionate, flexible, and focused on real-life needs—because every young person deserves the chance to heal and grow.
